Determining Health Rate Class

Insurance companies determine premiums based on several factors, including health and risk related issues. Information below outlines general guidelines that most insurance companies use to classify customers, the four major classifications are:

Preferred Plus, Preferred, Standard plus, and Standard.

If you do not meet one or more of the requirements listed in the Preferred plus, Preferred, or Standard plus columns, please select "Standard" when calculating your quotes.

If you have a health impairment, not listed here, insurance rates higher than standard may apply.

Physical/Mental Impairments and Medical History:
  • Best Class/Preferred Plus

  • No health impairments allowed. No personal history of cardiovascular disease or diabetes.
  • Preferred

  • No health impairments allowed. No personal history of cardiovascular disease or diabetes.
  • Standard Plus

  • Some minor health impairments allowed.
  • Standard

  • Some health impairments allowed. Can have personal history of certain medical conditions.

Blood Pressure:

  • Best Class/Preferred Plus

  • No current or past history of blood pressure treatment. Current and average readings in the past two years not greater than 140/85.
  • Preferred
    Currently well controlled with current and average readings in the past two years not greater than: 150/90
  • Standard Plus
    Currently well controlled with current and average readings in the past two years not greater than: 150/90 - age 60 and younger, 155/95 - age 61 and over.
  • Standard
    Currently well controlled with current and average readings in the past two years not greater than: 155/95 - age 60 and younger, 160/95 - age 61 and over.

Family Medical History:

  • Best Class/Preferred Plus

  • No cardiovascular disease or cancer in either parent or siblings before age 61.
  • Preferred
    No cardiovascular death of either parent before age 61.
  • Standard Plus
    No more than one cardiovascular death in parents before age 61.
  • Standard
    No more than one cardiovascular death in parents before age 61.

Personal Driving History:

  • Best Class/Preferred Plus

  • Not available if 2 or more moving violations in last 3 years, or if any DUI in last 60 months.
  • Preferred
    Not available if 3 or more moving violations in the last 3 years except with flat extra, or if any DUI in last 60 months.
  • Standard Plus
    Not available if 3 or more moving violations in the last 3 years except with flat extra, or if any DUI in last 24 months.
  • Standard
    Not available if 3 or more moving violations in the last 3 years except with flat extra, or if any DUI in last 24 months.

Use of Tobacco or Nicotine Products:

  • Best Class/Preferred Plus

  • No use in the past 36 months.
  • Preferred
    No use in the last 12-36 months (varies by company).
  • Standard Plus
    No use in the last 12-24 months (varies by company).
  • Standard
    No use in the last 12 months.

History of Cancer:

  • Best Class/Preferred Plus

  • Depending on company, only available on certain types of skin cancer.
  • Preferred
    Only available on certain types of skin cancer.
  • Standard Plus
    Coverage may be available based on specific cancer history.
  • Standard
    Coverage may be available based on specific cancer history.

Cholesterol Readings:

  • Best Class/Preferred Plus

  • Levels may not exceed 220.
  • Preferred
    Levels may not exceed 250.
  • Standard Plus
    Levels may not exceed 285.
  • Standard
    Levels may not exceed 300.

Cholesterol / HDL Ratio:

  • Best Class/Preferred Plus

  • May not exceed 4.0-5.0 (varies by company).
  • Preferred
    May not exceed 6.0-6.5 (varies by company).
  • Standard Plus
    May not exceed 7.0 (varies by company).
  • Standard
    May not exceed 7.5-8.0 (varies by company).

Substance or Alcohol Abuse:

  • Best Class/Preferred Plus

  • No history of substance or alcohol abuse.
  • Preferred
    No history of, or treatment for, abuse in past 10 years.
  • Standard Plus
    No history of, or treatment for, abuse in past 7 years.
  • Standard
    No history of, or treatment for, abuse in past 7 years.

Aviation:

  • Best Class/Preferred Plus

  • Commercial airline pilots accepted. Private pilots - may qualify with aviation exclusion.
  • Preferred
    Commercial airline pilots accepted. Private pilots- flat extra may apply depending on experience.
  • Standard Plus
    Commercial airline pilots accepted. Private pilots- flat extra may apply depending on experience.
  • Standard
    Commercial airline pilots accepted. Private pilots- flat extra may apply depending on experience.

Hazardous Activities (i.e. Sky diving, Hang gliding, Rock climbing, Scuba diving etc):

  • Best Class/Preferred Plus

  • Available only if no flat extra premium would be required.
  • Preferred
    Available; however, may have flat extra.
  • Standard Plus
    Available; however, may have flat extra.
  • Standard
    Available; however, may have flat extra.

Residency-Citizenship:

  • Best Class/Preferred Plus

  • Must be a US resident for past 3 years and either a US citizen or have permanent Visa or Greencard.
  • Preferred
    Must be a US resident for past 3 years and either US citizen or have permanent Visa or Green card.
  • Standard Plus
    Must be a US resident for past 2 years and either US citizen or have permanent Visa or Green card.
  • Standard
    Must be a US resident for past 2 years and either US citizen or have permanent Visa or Green card.
